Important Notes for the Premium-Free "Annual Dental Insurance Plan":

1. To enjoy the "Annual Dental Insurance Plan" at no premium, Cardmember is only required using his/her CCB (Asia) Credit Card at least once per statement cycle or has a total accumulated spending of a sum equals to or exceeding HKD12,000 during the effective Cardmembership year, included retail purchases and cash advances.
2. However, if a Cardmember has, at any time, redeemed the service and cannot comply with the above Card usage requirement, an administration fee of HKD500 will be charged to the Cardmember's Card Account by China Construction Bank (Asia) Corporation Limited (the "Bank") at the end of the Cardmember's Card membership year or upon termination of the Cardmember's Credit Card (whichever is earlier).
3. If a Cardmember chooses to renew the Annual Insurance Plan(s) upon the annual renewal date of his/her Card membership, an enrolment fee will be charged. The enrolment fee for the "Annual Dental Insurance Plan" is HKD60.
4. The above insurance plans are underwritten by AIA International Limited (Incorporated in Bermuda with limited liability).
5. The enrolment form for the above insurance plans will be mailed to each Cardmember together with the "Cardmember Privileged & Benefits" Leaflet, and the new credit card.
6. The terms and conditions of the above insurance plans will be mailed to each Cardmember within 4 weeks upon the Bank's receipt of the enrolment form.
7. The Bank reserves the right to revise the fees and charges of the above insurance plans at any time and without prior notice.
